While Tina's daughter tells her Roadhouse friend about Billy's last sighting, she mentions that Billy was bleeding from his nose and mouth. The "Drunk" in lockup at the Sheriff Department is also bleeding from the mouth and face. But curiously he is left in his cell after Andy brings everyone (James, Naido, Green Glove) up to Sheriff Truman's office for the final showdown with Bob. Is this not Billy after all? And if it is, why introduce this character multiple times just to leave him out of the "I hope to see you all again" group?

For awhile I wondered if he was Dougie because he repeated words like Dougie did....
Highly doubt it's Billy. The drunk's purpose seemed to be to keep Chad in check and it also seemed that only Chad could see him. Also, the fluid dripping from his mouth was dark brown. Could be blood mixed with scorched oil or tobacco. Not really sure. Probably a minion of Mike.

I agree. We don't see any of the police interact with him. If he were really in that bad of shape, certainly they would have gotten him treatment beyond what he received. I think he was from the lodge to keep Chad off balance, and perhaps to protect Naido. He mimics Naido, so perhaps she understood, and he may have been telling her what was happening because she was unable to see. Maybe his sounds were meant to comfort her.

Agreed. He was just there to torment Chad for whatever reason. I'm cool with that. Chad sucked. 🙂

My personal feeling is that he isn't a lodge spirit, but simply a drunk who is bleeding unrealistically. Maybe he's been influenced by the black lodge, much like the young girl in the car who looked like a zombie.
I personally think he was added for dark, directorial impact, with no other story or reason.

The reasons I thought he might be a positive force was the fact that he worked against Chad, and seemed sympathetic to Naido. Of course, I have no real idea.
I interpreted it similarly, but yeah, My0k could be correct as well. Let's ask Lynch (Frost may not even know. Haha).
Makes sense. Explains the lack of interaction with the police and his lack of air time after Chad's grand escape/concussion.

Lynch usually gives lodge spirits some presence. That guy had the presence of some random dude who was bleeding and under the influence.
Billy did seem zombified like the kid in the car who Bobby encountered. There was no real follow up on that or Billy.
But the fact that Andy didn't bring Billy up to Truman's office suggests that Billy is either 'not real' (and maybe a lodge plant like people have said) or Andy is a crappy cop haha. For plot purposes it would have been weird and distracting to have Billy bleeding in Truman's office though. So they ignored it.
IF Billy is lodge-related, he may be neither fully good or bad. We've seen that lodge actors often have mixed or opaque motives. That Chad might have his own tormentor is not impossible.
Weird characters in Lynch films aren't exactly a clear sign of supernatural influence.
