Maybe you're right. It was hard to tell on my TV whether that was supposed to be dandruff or like a glittering light such as Cooper has seen in the Case Files and on Anthony's face previously. It was just dandruff, huh? Was it a visual clue for Cooper to make his mind subconsciously think "poison?" If so, what does dandruff or poison have in it as a visual cue to make Cooper—the man who can't get out of a car on his own—reach out and massage Anthony's shoulders? Was it the coffee working?

Wow. I never could have thought of this, but it is very convincing. It seemed different when he took down Ike, though. It looked like Agent Cooper's instinct and training kicked right in. The arm showed up with some additional instructions to tear Ike's hand up, but the initial moves looked like vintage Agent Cooper.

There are quite a few acupressure/puncture points there - and they overlap with the fibromyalgia 'tender points' which are described as being 'exquisitely painful'. (I can vouch for that!)
I've had acupuncture in those points and after a bit of screaming/swearing/biting the trolley the muscles relax & you feel really euphoric. I'm not a believer in chi & all that, but something strange definitely happens! If you don't actually have chronic pain, I imagine it would be relaxing - especially someone who hasn't been sleeping etc - as Tony described.
