Did you see the subtitles in Miriams scene? The read as follows:
[Door opens]
[Footsteps tapping]
[Door shuts]
[Monitor beeping]
This happens so quickly and I only have subtitles up on my computer, so I totally missed it.
So, someone sneaks in and kills Miriam in this scene?
Sounded more like busy hospital sounds. Presumably, she had already implicated Richard (though it's funny that no one actually says the information came from her, like no one ever mentions present-day Audrey).
At the very end of the scene it sounds like an alarm going of. Possibly from one of the monitors.

It just sounded like some other machine beeping, not an alarm. The heart monitor or whatever Miriam's hooked up to keeps the same rhythm throughout. And the scene ends too quickly for any particular plot development.
