I took that as referring to the same kind of thing that happened to his face when he looked in the mirror in prison and said to BOB, "Good. You're still with me."

I think you've picked up on the creepiest part - that was the real shudder moment. We didn't need any graphic details to know how awful it was. Or was it? This was a 'fake' Diane. Doppel or tulpa - was she deliberately trying to provoke emotion? Did that really happen - or was it a convenient backstory?
One just wonders about that smile when Sarah took her face off. That was a prerry damned scary smile.

Yes I watch with closed captioning on and it said "jaw cracks" or something like that
"I'm not me!" as she pulled out the gun. I took that as an intended warning, she did the equivalent of "cop suicide" ( draw a weapon on them, or make cops believe you are to get them to shoot you ).
All good theories above: garmonbozia, Bob smiling, the Sarah Cheshire Cat smile (which might be a manifestation of Bob).
But I thought that Laura Dern was doing a fake acting thing, especially when she finally says "he raped me." I'm not an expert in what testimonies on the subject sound like, but it sounded like an actor portraying a false testimony. Maybe she knew she was a tulpa and was doing what she was supposed to do.... When she first got the message and said "I remember" and sent off that long number, it seemed to me like she was remembering her task and how to do it, not that an old memory of an event had returned.
